Black Moon Lilith is not a physical body but a mathematical point: the empty focus of the Moon's elliptical orbit around Earth, marking where the Moon sits furthest away. In a natal chart it is read as a marker of raw, unapologetic desire and the parts of a person that resist being tamed or made convenient for others. It matters because, unlike the planets, it points to instinct that has usually been suppressed or shamed at some stage.
